Immunization Clinic for Students.

Horn is sponsoring a immunization clinic for students on October 2nd: 9-2:00pm.   In most cases the cost is free if the student has no insurance, insurance is accepted.   The forms are attached below that must be completed and turned in to Mrs. Ballard or myself, Nurse Powell by September 25th.  Please share this with your students and remind your Seniors that the meningitis is required if they are planning to go to college.  If you have questions or would like the printed form you may reach out to Mrs. Ballard or Nurse Powell.
